The Operational Team Lead Integrated Care ICPCD Specialist Community Team will play a key role in implementing the model of care for the prevention and management of chronic disease within the CHO.  The post holder is responsible for coordination of ICPCD Specialist team and Hub operations, ensuring the provision of a seamless integrated service and multidisciplinary input for patients with chronic disease i.e. Diabetes, Respiratory or Cardiac.  The post holder will support the development of the new clinical roles and functions within the Specialist Community Team.
